On the snowy evening of February 12, 2009, Continental Connection Flight 3407 -- operated by regional carrier Colgan Air -- fell out of the sky and crashed in Clarence Center, New York.

The next morning I was on the first plane to Buffalo, and what I saw at the crash site a few miles from my house is something I'll never forget. Charred earth and twisted metal. Grieving families of the 50 victims, including a pregnant woman. Bone-tired first responders recovering human remains.

It's a sight no one should have to see, and today, a year and a half later, we know what happened, why, and how to prevent it in the future.
